PEAK Application

Once you have submitted your preliminary contact information (below), you will be taken to the application page. You will also be emailed a link to your application, allowing you to exit and re-enter the application as needed. Please fill out the application form in its entirety and submit in addition to:

• High school transcripts
• Vaccination records (or request a vaccination waiver from the Admissions Office)

Once admitted, a $100 deposit will be required to hold your place in the program.
